Job summary

Optimal Hospice seeks a Registered Nurse Case Manager to deliver patient-centered hospice care. You will perform comprehensive assessments, manage plans of care, and coordinate services with the interdisciplinary team to ensure comfort and quality of life for patients and families.

Responsibilities include administering treatments, counseling families, maintaining documentation, and acting as a liaison with physicians to adapt plans as patient needs evolve.

Qualifications

• Registered nurse with current licensure to practice professional nursing in the state.
• Maintains a current CPR certification.
• Strong assessment and care planning capabilities.
• Experience collaborating with interdisciplinary teams.

Responsibilities

• Completes initial, comprehensive and ongoing assessments of patients and families.
• Provides professional nursing care using the nursing process.
• Develops and revises care plans with patient and family involvement.
• Administers medications and treatments as prescribed.
• Coordinates patient care as Case Manager with Clinical Supervisor.
• Keeps accurate clinical notes and communicates changes to the physician.
