Job summary

Gentiva Hospice is seeking an experienced Registered Nurse to join our team as an RN Case Manager, delivering high-quality hospice care in patients' homes across Spencer, TN and surrounding areas.

You will perform comprehensive assessments, develop and adjust plans of care, coordinate with an interdisciplinary team, educate families, and maintain clinical documentation in accordance with regulatory standards and quality initiatives.

Qualifications

• Active RN license in the state of employment.
• Minimum 1 year RN experience in acute care, hospice or home health.
• Hospice or palliative care experience preferred.

Responsibilities

• Perform comprehensive patient assessments and reassessments.
• Deliver skilled nursing interventions per Plan of Care.
• Coordinate care across IDG and refine care plans.
• Provide education and bereavement guidance to families.
• Document timely clinical records and ensure regulatory compliance.
