Gentiva Hospice is seeking an experienced Registered Nurse to join as an RN Case Manager. You will be the primary contact for patients and families, delivering skilled hospice nursing care in home settings, updating plans of care, and coordinating with an interdisciplinary team to ensure compassionate, patient-centered end-of-life care.

Strong assessment and documentation skills are essential. You will educate families, manage symptoms, and support caregivers, with a focus on dignity and comfort

Qualifications

• RN with at least 1 year of RN experience in hospice, home health, or related acute settings.
• Current RN license to practice in the state of employment; CPR certified.
• Knowledge of hospice/palliative care concepts and regulations.
• Strong assessment, communication, and documentation skills.

Responsabilités

• Perform comprehensive patient assessments and reassessments, including in-person and telephonic visits, and educate patients, families and caregivers as appropriate.
• Deliver nursing interventions and symptom management aligned with goals of care.
• Develop and update individualized plans of care with visitation schedules and risk identification.
• Coordinate care across IDG, consulting physicians, and other clinicians.
• Provide patient and caregiver education on disease process, end-of-life care, medications, and self-care.
