Job summary

Hospice of New Jersey is seeking a full-time Hospice RN Case Manager (RNCM) to join our nursing team. You will provide hands-on hospice nursing care and coordinate services across the community with a compassionate approach.

As part of our interdisciplinary team, you will educate patients and families, guide care plans, and ensure comfort and dignity at the end of life. Requires RN license and 1 year med-surg/acute care experience.

Qualifications

Hold current unencumbered RN license.
Minimum 1 year RN experience in med-surg/acute care; hospice a plus.
Proficient with computers and EMR systems.
CPR certification (per state requirements).
Valid driver’s license and reliable transportation.

Responsibilities

Deliver hands-on hospice nursing care and coordinate services.
Collaborate with Clinical Director to assess client eligibility for hospice services.
Educate patients and families, empowering active participation in care plans.
Provide compassionate, skilled nursing to support comfort and quality of life.
Participate in call duties and team meetings.
Apprise and coordinate with interdisciplinary care team for appropriate care.
