Job summary

Legacy Hospice is seeking a compassionate Registered Nurse Case Manager (RNCM) to provide high-quality end-of-life care in patients' homes. You will deliver comprehensive assessments, develop care plans, and coordinate with an interdisciplinary team to ensure compassionate, dignified support for patients and families.

The role requires an active RN license, 1 year hospice/home health experience, CPR/BLS, and a valid driver's license.

Qualifications

• Valid RN license in the state of employment.
• BSN or CHPN preferred.
• Minimum 1 year of nursing experience in hospice, home health, or related field.
• Strong interpersonal, clinical, and organizational skills.
• Active CPR/BLS certification.
• Valid driver’s license and reliable transportation.
• Ability to work independently and with a team.
• Pre-employment drug screening including marijuana.

Responsibilities

• Perform comprehensive patient assessments and create individualized care plans.
• Manage a caseload of hospice patients and coordinate care with the IDT.
• Administer medications and treatments per physician orders.
• Educate patients and families on symptom management and end-of-life care.
• Monitor and update care plans based on ongoing assessments.
• Provide emotional and clinical support to patients and caregivers.
• Communicate with physicians, care partners, and community resources.
• Maintain accurate, timely clinical documentation.
• Participate in IDT meetings and on-call rotation.
• Supervise ancillary staff and ensure continuity of care.
