Job summary

Lifted Hospice is seeking an experienced Registered Nurse (RN) Case Manager to deliver expert care in patients’ homes or their care settings. Monitor symptoms, guide care plans with the interdisciplinary team, and educate families to help them understand expectations and comfort during end-of-life care.

You will lead bedside care, coordinate with social workers, physicians, and the medical director, and ensure compassionate, patient- and family-centered care that honors life.

Qualifications

• Graduate of an accredited Diploma, Associate, or Bachelor’s nursing program.
• Active state license and current clinical skills.
• Experience as a registered nurse in a clinical care setting; hospice experience preferred.
• Excellent communication and interpersonal skills.
• Must read, write and comprehend English.

Responsibilities

• Assessing needs to guide care based on physical, emotional, and clinical needs.
• Managing hospice symptoms to improve quality of life.
• Working with the interdisciplinary team to follow unified care plans.
• Keeping accurate records in line with standards and requirements.
• Supporting families with education, emotional support, and confidence-building.
