Job summary

Legacy Hospice, Inc. is seeking a compassionate Registered Nurse Case Manager (RNCM) to deliver high-quality end-of-life care to patients in their homes in Independence, Missouri. The role supports families with empathy and respect, working within an interdisciplinary team and on-call rotation.

The RNCM will assess patients, coordinate care, educate families on symptom management, and maintain thorough clinical documentation while providing emotional support to patients and caregivers.

Qualifications

• Must hold an RN license in the state of employment.
• BSN or CHPN preferred.
• Minimum 1 year of hospice/home health nursing experience.
• Active CPR/BLS certification and valid driver’s license.
• Drug-free workplace; pre-employment drug screening including marijuana.

Responsibilities

• Conduct comprehensive patient assessments and develop care plans.
• Manage a caseload and coordinate with the IDT.
• Administer medications and treatments per physician orders.
• Educate patients and families on symptom management and end-of-life care.
• Maintain timely clinical documentation and participate in IDT meetings.
• Provide support to patients and caregivers; participate in on-call rotation.
• Supervise ancillary staff and ensure continuity of care.
