Job summary

Peaceful Hearts Hospice seeks a compassionate Registered Nurse Case Manager to oversee a patient caseload, assess needs, develop plans of care, and coordinate with physicians and the interdisciplinary team to deliver dignified hospice care across settings.

Applicants should be licensed RNs, with hospice or related experience, and able to travel as needed. The role includes on-call coverage, supervision of staff, charting in EMR, and supporting families through end-of-life care.

Qualifications

• Current, unencumbered RN license in Nebraska.
• Graduate of an accredited school of Nursing.
• Experience in hospice, home health, or oncology preferred.

Responsibilities

• Own a patient caseload, assess and plan nursing care.
• Coordinate with physicians and the IDT to ensure quality care.
• Provide skilled nursing services and patient education.
• Supervise LPNs and Hospice Aides and document care observations.
• Participate in 24/7 on-call coverage and timely charting.
