Job summary

Vitality Hospice & Palliative Care is seeking a Registered Nurse Case Manager (RNCM) to join our compassionate team. You will assess patients, develop plans of care, collaborate with the IDT, and coordinate home visits across the service area.

The role requires an NJ RN license, hospice or home health experience, strong communication, a valid driver's license, and willingness to travel. We offer a comprehensive benefits package and flexible scheduling.

Qualifications

Current Registered Nurse (RN) License in New Jersey (required).
Clinical nursing experience (hospice or home health preferred).
Strong communication, organizational, and interpersonal skills.
Valid driver’s license, reliable vehicle, and current auto insurance.
Ability to travel within the assigned service area.

Responsibilities

Conduct patient admissions and ongoing nursing assessments.
Develop and implement individualized plans of care based on each patient's needs and goals.
Collaborate closely with hospice physicians and the interdisciplinary team (IDT).
Supervise and coordinate care with home health aides, social workers, and chaplains.
Educate patients and caregivers about disease progression, symptom management, and available resources.
Ensure timely and accurate documentation in compliance with hospice standards.
Promote a culture of excellence, empathy, and accountability in every interaction.
