Job summary

Grace Hospice seeks an experienced RN Case Manager to deliver end-of-life hospice care in patients’ homes or community settings. You will supervise hospice aides, coordinate care, and ensure compassionate, patient-centered support for patients and families.

The role requires strong clinical judgment, excellent communication, and the ability to navigate sensitive, meaningful experiences with empathy and professionalism.

Qualifications

• Current unrestricted RN licensure in the state of practice.
• Two years of RN experience.
• Two years of Hospice/Palliative Nursing experience preferred.
• Valid driver’s license and proof of auto insurance.

Responsibilities

• Provide end-of-life hospice care in patients’ homes or community settings.
• Supervise and instruct hospice aides in personal patient care; coordinate with the team.
• Maintain care coordination notes and ensure compliant documentation.
• Assist with medication and supply delivery after hours, weekends, and holidays.
• Attend interdisciplinary team meetings during on-call periods for patient needs.
