Job summary

Affinity Hospice Management LLC seeks a Registered Nurse to plan, direct, and coordinate hospice care in patients' homes and communities. The RN will perform comprehensive assessments, develop and revise plans of care, and educate patients and families while collaborating with an interdisciplinary team to ensure compassionate end-of-life care.

The role requires NLN-accredited nursing education, current state RN licensure, and experience in home health/hospice.

Qualifications

• Graduate of National League of Nursing accredited school of nursing.
• Current licensure in State.
• 1 year of nursing experience within last 3 years or BSN with 2 years’ experience in public health, home care, or hospice preferred.
• Management experience not required; responsible for supervising hospice aides.

Responsibilities

• Perform comprehensive patient and family assessments to determine hospice needs.
• Develop and initiate care plans; revise as patient needs change.
• Administer medications and treatments per physician’s plan of care.
• Counsel patient and family on nursing and related needs; provide health instructions.
• Coordinate patient care with the interdisciplinary hospice team; attend meetings.
• Participate in on-call duties and ensure equipment/services availability.
• Supervise ancillary personnel and pursue ongoing professional development.
