Job summary

Gentiva is seeking an experienced Hospice RN / Case Manager to lead in-home hospice care and family education across Michigan. You will perform assessments, deliver skilled nursing care, and collaborate with a multidisciplinary team to refine personalized care plans.

Ideal candidates hold a current RN license, with at least 1 year in hospice/palliative settings. CPR, driver’s license, and auto insurance are required for field visits. Growth opportunities and competitive benefits are offered.

Qualifications

• Current RN license in state of employment.
• Hospice or palliative care experience preferred.
• CPR certification required; valid driver's license and auto insurance.

Responsibilities

• Perform comprehensive patient assessments and reassessments, providing education to patients, families and caregivers.
• Deliver skilled nursing interventions and symptom management per plan of care.
• Develop and update individualized plans of care including visit frequency.
• Coordinate care across IDG, including physicians and nurses.
• Provide education on disease processes, self-care, end-of-life care, medications, nutrition.
