Job summary

Gentiva is seeking a dedicated Hospice RN / RN Case Manager to lead patient care in home settings, coordinating with an interdisciplinary team to deliver personalized end-of-life care. You will perform RN assessments, manage symptoms, and guide families through the hospice journey.

The role requires an active RN license in Minnesota and at least one year of RN experience in hospice or related fields. CPR certification and a valid driver’s license are required, with comprehensive benefits for

Qualifications

• Hospice or palliative care experience preferred.
• One year RN experience in acute care, oncology, long-term care, hospice or home health.
• Current RN license in the state of employment and CPR certification.
• Valid driver’s license and automobile insurance.

Responsibilities

• Perform comprehensive patient assessments and reassessments in home settings and via telephonic/virtual visits.
• Deliver skilled nursing interventions and symptom management per Plan of Care.
• Develop and update individualized care plans and adjust visit frequency as needed.
• Coordinate with IDG including physicians and other nurses to refine care plans.
• Educate patients and caregivers on disease process, medications, and end-of-life care.
• Communicate condition changes to the care team and document findings promptly.
• Support continuity of care across home health and hospice referrals as appropriate.
• Maintain accurate clinical documentation and comply with infection control standards.
• Participate in on-call and patient support activities as applicable.
