Qualifications

• Current RN license in the state of employment.
• CPR certification required.
• Minimum of 1 year RN experience in hospital, hospice or home health.
• Hospice or palliative care experience preferred.

Responsibilities

• Perform comprehensive patient assessments and reassessments, educate patients and caregivers.
• Deliver skilled nursing interventions and symptom management.
• Develop and update individualized care plans with visit frequency.
• Coordinate care with physicians and IDG members.
• Provide education on disease, self-care, and end-of-life care.
• Communicate changes in condition to the care team and ensure timely interventions.
• Document clinical records accurately and maintain standards of infection control and compliance.
• Support continuity of care and transition across care levels.
• Participate in on-call and patient support activities.
