Job summary

Qualifications

• RN with current license in the state of employment.
• Minimum 1 year RN experience; hospice/palliative care preferred.
• CPR certification required; valid driver’s license and auto insurance.

Responsibilities

• Assess patients and provide education to families.
• Deliver nursing interventions and symptom management.
• Develop and update individualized plans of care.
• Coordinate care with physicians and IDG.
• Educate on disease process, end-of-life care, medications.
• Communicate changes in condition to the care team.
• Document timely clinical records and maintain compliance.
• Participate in on-call and patient support activities.
