Job summary

Luminary Hospice is seeking a Registered Nurse Case Manager to plan, direct, and coordinate hospice care in patients' homes and communities. The role emphasizes community health education and seamless collaboration with interdisciplinary teams to deliver compassionate, high-quality end-of-life care.

The nurse will perform comprehensive assessments, develop and revise care plans, administer treatments, and educate patients and families while supporting bereavement and emotional needs.

Qualifications

• Graduate of an accredited school of nursing; 1–2 years of recent acute care and 1–2 years of home care preferred.
• Current licensure in state and CPR certification.
• Bachelor’s degree with hospice care experience preferred.
• Management experience is not required; may supervise hospice aides.
• Excellent observation, verbal and written communication skills, problem-solving skills, basic math skills.
• Ability to lift, position, and transfer patients; valid driver’s license and insured vehicle.

Responsibilities

• Completes initial and ongoing comprehensive assessments of patient and family to determine hospice needs.
• Provides professional nursing care using the nursing process.
• Writes and initiates a plan of care; re-evaluates needs with patient and family.
• Initiates the plan of care and revises as patient status changes.
• Uses health data to determine nursing diagnosis; develops goals with patient/family involvement.
• Administers medications and treatments as prescribed; provides patient/family education.
• Acts as Case Manager coordinating patient care for assigned caseload.
• Completes and submits clinical notes; communicates with physician and care team.
• Sends and receives orders; coordinates with community health providers.
• Participates in on-call duties; ensures equipment and services are available.
• Supervises ancillary personnel when required; pursues ongoing professional development.
• Participates in quality improvement activities.
