Job summary

Luminary Hospice is seeking a Registered Nurse Case Manager to plan, organize, and direct hospice care across patients and families in home and community settings.

The role emphasizes nursing leadership, care coordination, and ongoing assessments to develop and revise plans of care in collaboration with an interdisciplinary team. A current RN license and CPR are required, with hospice experience preferred.

Qualifications

• Graduate of an accredited school of nursing.
• Current licensure in State with CPR certification.
• Bachelor’s degree with hospice care experience preferred.

Responsibilities

• Conduct comprehensive patient assessments to determine hospice needs.
• Provide professional nursing care and administer medications per physician's plan.
• Coordinate patient care as Case Manager with interdisciplinary team.
• Maintain accurate clinical notes and communicate with physicians.
• Assist with on-call duties and supervision of ancillary personnel.
