Job summary

Luminary Hospice seeks a Registered Nurse Case Manager to plan, organize, and direct hospice care in patients’ homes and communities. You will build from community resources to provide comprehensive, patient- and family-centered care with an emphasis on comfort and dignity.

Responsibilities include conducting assessments, developing care plans, coordinating interdisciplinary care, and educating families. Strong nursing skills, licensure, and CPR are required.

Qualifications

• Must be a graduate of an accredited school of nursing.
• Current licensure in state and CPR certification.
• Bachelor’s degree; hospice care experience preferred.
• Excellent observation, verbal and written communication skills, and problem-solving abilities.

Responsibilities

• Conduct comprehensive initial and ongoing assessments of patients and families to determine hospice needs.
• Develop and revise plans of care with the interdisciplinary team.
• Administer medications and treatments as prescribed by the physician.
• Coordinate patient care across home and community settings to meet patient needs.
• Educate patients and families on care and self-care techniques; ensure safety and quality of care.
