Job summary

Luminary Hospice is seeking a Registered Nurse Case Manager to plan, organize, and direct hospice care in patients' homes and communities. You will build from community resources to meet patient and family needs, coordinating care with an interdisciplinary team.

The role emphasizes nursing expertise, patient education, and plan-of-care development, with responsibilities spanning assessment, care planning, and family counseling in a compassionate hospice setting.

Qualifications

• Graduate of an accredited school of nursing.
• Current licensure in state and CPR certification.
• Bachelor’s degree, with hospice care experience preferred.
• Management experience is not required; may supervise aides.
• Excellent observation, verbal and written communication; problem-solving.
• Ability to lift, move, and transfer patients up to 50 pounds.

Responsibilities

• Completes comprehensive assessments of patient and family to determine hospice needs.
• Provides professional nursing care using the nursing process.
• Develops and revises a plan of care with patient and family involvement.
• Administers medications and treatments as prescribed.
• Counsels patient and family; provides health care instructions.
• Assists with activities of daily living and supports self-sufficiency.
• Acts as Case Manager and coordinates care for assigned caseload.
• Maintains clinical notes and communicates with physicians and care team.
• Participates in on-call duties and interdisciplinary team meetings.
