Job summary

Luminary Hospice is seeking a Registered Nurse Case Manager to plan, organize, and direct hospice care in patients’ homes and communities. The role emphasizes nursing excellence, patient education, and collaboration with the interdisciplinary team to deliver comprehensive end-of-life care.

The ideal candidate has a nursing degree, valid Ohio licensure, CPR certification, and experience in home health or hospice settings.

Qualifications

• Graduate of an accredited school of nursing.
• Current licensure in State and CPR certification.
• Bachelor’s degree, hospice care experience preferred.
• Excellent observation, verbal and written communication skills.
• Ability to work effectively in patients’ homes and communities.

Responsibilities

• Completes initial, comprehensive assessment of patient and family to determine hospice needs and develops a plan of care.
• Provides professional nursing care using the nursing process and monitors patient status.
• Coordinates care with physicians and the interdisciplinary team to ensure continuity and quality.
• Teaches patient and family self-care techniques and supports emotional and spiritual needs.
