Job summary

Luminary Hospice is seeking a skilled Registered Nurse Case Manager to coordinate and deliver home-based hospice care. You will assess patients, plan and implement nursing care, and work with families to support comfort and dignity at end of life.

The role requires Ohio RN licensure, CPR certification, and strong communication. You will collaborate with an interdisciplinary team to ensure high-quality, compassionate care in patients’ homes and communities.

Qualifications

Graduate of an accredited school of nursing.
Current licensure in State of Ohio (RN).
CPR certification and strong communication skills are required.

Responsibilities

Conduct initial and ongoing comprehensive patient and family assessments.
Develop and revise plans of care in collaboration with the interdisciplinary team.
Provide professional nursing care and administer medications as prescribed.
Educate patients and families on disease process, pain management, and care at home.
Coordinate patient care across home health services and community resources.
Participate in on-call duties and ensure timely documentation.
