Job summary

Luminary Hospice is seeking a PRN Registered Nurse Case Manager to plan, organize, and direct hospice care in home settings across Texas. You will build plans of care, collaborate with physicians, and coordinate with the interdisciplinary team to meet patient needs.

The role emphasizes community health nursing, patient and family education, and on-call support, with a focus on maintaining dignity and comfort for end-of-life patients and families.

Qualifications

• Graduate of an accredited school of nursing.
• Current RN license in Texas and CPR certification.
• Bachelor’s degree preferred; hospice/home care experience 1–2 years preferred.

Responsibilities

• Completes initial and ongoing comprehensive assessment of patient and family to determine hospice needs.
• Develops and revises a plan of care with patient/family involvement.
• Delivers professional nursing care per the physician’s plan of care.
• Assists patient with ADLs and supports self-care and comfort goals.
• Communicates with physicians and coordinates care with the interdisciplinary team.
• Participates in on-call duties and ensures necessary equipment/services are available.
