Job summary

Luminary Hospice is seeking a Registered Nurse Case Manager to lead patient-centered hospice care in home and community settings. You will perform comprehensive assessments, develop and revise plans of care, and coordinate services with the interdisciplinary team to meet patient goals and comfort needs.

Requirements include active RN license, CPR, 1–2 years in acute care and hospice/home care preferred, strong communication with patients and families, and a commitment to compassionate

Qualifications

Graduate of an accredited nursing program with current state licensure.
1–2 years of acute care and 1–2 years hospice/home care experience preferred.
CPR certification required.
Excellent communication and patient-family education skills.

Responsibilities

Completes an initial, comprehensive assessment of patient and family to determine hospice needs.
Develops and initiates a plan of care; revises plan as patient status changes.
Acts as Case Manager coordinating patient care for assigned caseload and communicates with physicians.
Documents clinical notes and updates on pain/symptom management.
Coordinates care with the interdisciplinary team and conducts family education.
Participates in on-call duties as defined by policy.
