Job summary

Gentiva Health Services, Inc. is seeking a compassionate Hospice RN / RN Case Manager to lead patient care at home and coordinate with an interdisciplinary team in Wisconsin.

You will assess patients, deliver skilled nursing care, document plans of care, mentor caregivers, and ensure comfort, dignity, and clear communication for families navigating end-of-life options. This is a full-time role with on-site home visits and strong team collaboration.

Qualifications

• Current Wisconsin RN license.
• Minimum of 1 year RN experience; hospice or home health preferred.
• CPR certification is required.

Responsibilities

• Perform comprehensive patient assessments and ongoing reassessments with education for families.
• Deliver skilled nursing interventions and symptom management per care plans.
• Develop and update individualized plans of care and visit schedules.
• Coordinate care across IDT and consult with physicians and other providers.
• Educate patients and caregivers on disease, medications, self-care, and end-of-life options.
• Communicate changes in condition promptly to the care team and document thoroughly.
• Support continuity of care across home health, palliative, and hospice referrals; participate in on-call duties.
