Job summary

Gentiva is seeking a compassionate Hospice RN / RN Case Manager to lead in-home end-of-life care. You will assess patients, implement and document plans of care, and coordinate with an interdisciplinary team to deliver comfort, dignity, and support to patients and families.

Ideal candidates have at least 1 year of RN experience, CPR certification, and a passion for patient-centered care in home health and hospice settings. This is a full-time position offering competitive pay and benefits.

Qualifications

RN license in the state of employment.
Minimum of 1 year RN experience in hospice, home health, or acute care.
Hospice or palliative care experience preferred.

Responsibilities

Perform comprehensive patient assessments and reassessments in home or facility settings.
Deliver and document skilled nursing care per Plan of Care.
Coordinate care with interdisciplinary team and provide education to patients and caregivers.
Maintain accurate clinical documentation and ensure regulatory compliance.
