Job summary

Gentiva is seeking a dedicated Hospice RN / RN Case Manager to provide high-quality care in patients' homes, assisted living, or other settings. You will lead assessments, develop care plans, and support families with education and bereavement guidance.

The role emphasizes interdisciplinary collaboration, accurate documentation, and adherence to infection control and regulatory standards. Ohio-based eligibility with a strong nursing background is required.

Qualifications

• RN licensure in good standing
• Minimum 1 year RN experience (hospice or home health preferred)
• Strong assessment and communication skills

Responsibilities

• Perform comprehensive patient assessments and reassessments
• Deliver nursing interventions and symptom management per plan of care
• Develop and update individualized care plans and visit frequency
• Coordinate care with interdisciplinary team
• Educate patients and caregivers on disease, end-of-life care, medications, nutrition
• Communicate changes in condition to care team
• Document clinical records accurately and timely
• Support IDG meetings and on-call patient support
