Job summary

Gentiva is seeking an experienced Hospice RN / RN Case Manager in Alabama to lead patient care in the home setting. You will perform comprehensive assessments, implement plans of care, and coordinate with physicians, social workers, and other team members to ensure compassionate end-of-life care.

You will educate families, manage symptoms, document care, and participate in on-call support as needed, delivering patient-centered services with a focus on dignity and comfort.

Qualifications

• RN license
• Minimum 1 year as an RN (acute care, hospice, or home health)
• Hospice or palliative care experience preferred
• Current CPR certification
• Valid driver’s license and auto insurance
• Knowledge of hospice and end-of-life care principles

Responsibilities

• Serve as primary contact for patients and families in hospice care
• Deliver and document skilled nursing care per Plan of Care
• Perform initial and ongoing patient assessments
• Collaborate with interdisciplinary team to refine care plans
• Provide emotional and spiritual support to patients and families
• Educate caregivers on safe home care
• Communicate patient condition updates to care team
• Maintain accurate clinical documentation and compliance
