Job summary

Luminary Hospice is seeking a Registered Nurse Case Manager to plan, organize, and direct hospice care in patients' homes and communities in Ohio. This role emphasizes community health education and coordination with families and an interdisciplinary team.

The ideal candidate will have nursing licensure in Ohio, CPR certification, and experience in home care or hospice settings. You will collaborate closely with physicians and care teams to ensure compassionate, comprehensive care.

Qualifications

• Graduate of an accredited school of nursing.
• Current licensure in the state of Ohio and CPR certification.
• Bachelor’s degree, with hospice care experience preferred.
• Experience in home health or community-based nursing is preferred.
• Excellent observation, verbal and written communication skills.
• Ability to lift and move patients and work in a home setting.

Responsibilities

• Conduct initial and ongoing patient assessments to determine hospice needs.
• Develop and revise care plans with the interdisciplinary team and family.
• Provide nursing care, administer medications, and monitor symptoms.
• Coordinate with physicians and caregivers to ensure continuity of care.
• Maintain clinical notes and communicate changes promptly.
• Participate in on-call duties and contribute to quality improvement.
