Job summary

Luminary Hospice is seeking a dedicated Registered Nurse Case Manager to coordinate home-based hospice care with a patient- and family-centered approach in the Dayton area. You will perform comprehensive assessments, develop care plans, administer treatments, and collaborate with an interdisciplinary team to ensure compassionate, high-quality end-of-life care for patients and their families.

Strong communication, clinical supervision, and organizational skills are essential to manage caseloads

Qualifications

Graduate of an accredited school of nursing.
Current licensure in State and CPR certification.
Bachelor’s degree, with hospice care experience preferred.

Responsibilities

Complete initial and ongoing comprehensive assessment of patient and family to determine hospice needs.
Provide professional nursing care by utilizing the nursing process.
Develop and revise the plan of care based on patient status and needs.
Initiate and administer medications and treatments as prescribed by the physician.
Counsel the patient and family in meeting nursing and related needs; provide health care instructions.
