Job summary

Gentiva is seeking an experienced Hospice RN/Case Manager to serve patients in their homes, assisted living, or other settings. You will lead comprehensive nursing care, perform initial and ongoing assessments, and collaborate with an interdisciplinary team to implement personalized plans of care.

You will educate patients and families, support end-of-life goals, and ensure clinical documentation meets regulatory standards.

Qualifications

• Registered Nurse licensed in the state of employment
• Minimum of one year RN experience, preferably in hospice, palliative care, or home health
• Hospice or palliative care experience preferred
• CPR certification required
• Current automobile insurance and valid driver’s license

Responsibilities

• Perform comprehensive patient assessments and ongoing assessments, including in-person and telephonic visits.
• Deliver skilled nursing interventions and symptom management aligned with goals of care.
• Develop and update individualized plans of care with appropriate visit frequency.
• Coordinate care across interdisciplinary teams including physicians and other clinicians.
• Provide patient and caregiver education on disease processes, end-of-life care, medications, nutrition, and self-care techniques.
• Document timely clinical records and maintain compliance with infection control standards.
• Participate in on-call and patient support activities as applicable.
