Qualifications

• Current unencumbered Ohio RN license.
• One year acute care nursing experience required.
• Experience in hospice preferred.

Responsibilities

• Provide ongoing assessment and nursing care to patients in homes and facilities.
• Follow nursing standards and hospice protocols.
• Provide care plans and symptom management for patient comfort.
• Communicate with other health care providers about changes in condition.
• Assess patient and caregiver needs and provide education as needed.
• Commit to high quality customer service for internal and external customers.
• Oversee implementation of the Plan of Care and evaluate effectiveness.
• Prepare for IDG by reviewing Plan of Care and reconciling medications.
