Job summary

Gentiva Hospice is seeking an experienced Registered Nurse Case Manager to provide home-based hospice care and education to patients and families. You will lead in assessments, symptom management, and coordination with physicians, social workers, and Chaplains to ensure compassionate, patient-centric care.

The role emphasizes care plan development, on-call responsiveness, and thorough documentation in a home health setting, aligning with quality standards and regulatory guidelines.

Qualifications

• Current license to practice nursing in the state of employment.
• Minimum of one year RN experience in acute care, oncology, long-term care, hospice or home health.
• Hospice or palliative care experience preferred.

Responsibilities

• Perform comprehensive patient assessments and reassessments and educate patients, families, and caregivers.
• Deliver skilled nursing interventions and symptom management per plan of care.
• Coordinate care across interdisciplinary teams and update plans of care.
• Document timely clinical records, orders, progress notes, and care plans.
• Participate in on-call and patient support activities as applicable.
