Job summary

Patriot Healthcare is seeking an RN Case Manager in South Carolina to coordinate and direct hospice services according to physician orders and patient plans, working closely with an interdisciplinary team to provide compassionate, high-quality care at life’s end.

The role requires an active SC RN license with nursing experience; hospice or long-term care experience is preferred. This is a full-time position offering benefits and a supportive, team-focused environment.

Qualifications

• Nursing degree from an accredited program and an active SC RN license.
• Hospice experience preferred; prior hospital/long-term care experience is helpful.

Responsibilities

• Coordinate and direct hospice services per physician orders and patient plans.
• Work with the Interdisciplinary Team (IDT) to ensure quality patient care.
• Document patient care activities accurately for billing and compliance.
• Supervise Hospice Aide care and CNA schedules as needed.
• Communicate with physicians and facility staff to coordinate care.
• Maintain up-to-date patient records and participate in quality assurance.
