Patriot Healthcare is seeking an RN Case Manager to coordinate and deliver hospice services in accordance with physician orders and patient plans. You will collaborate with the interdisciplinary team to ensure compassionate, high-quality care and proper documentation for billing and regulatory needs.

The role requires SC RN licensure and a nursing degree from an accredited program, with 1 year hospital or long-term care experience preferred.

Qualifications

• Must possess a Nursing Degree from an accredited college or university.
• Licensed as a Registered Nurse in the State of South Carolina.
• Experience: 1 year in a hospital, long-term care, or related health care facility preferred.

Responsabilités

• Provide, coordinate, and direct hospice services according to physician orders and patient plan of treatment.
• Serve as a member of the Interdisciplinary Team (IDT) and attend meetings.
• Perform administrative duties and ensure proper documentation for billing and regulatory compliance.
• Supervise Hospice Aide care and CNA schedules.
• Report occupational exposures per hospice policies.
