Job summary

Patriot Healthcare in South Carolina is seeking an experienced RN Case Manager to coordinate and direct hospice services in partnership with an interdisciplinary team. You will apply the nursing process to ensure comfort, dignity, and quality of life for patients and families.

The role requires an SC-licensed RN with prior hospital or long-term care experience; hospice experience is preferred. You will work with clinical staff to ensure adherence to policies and provide documentation that

Qualifications

• Must possess a Nursing Degree from an accredited college or university.
• Licensed as a Registered Nurse in the State of South Carolina.
• 1 year of nursing experience in hospital or long-term care preferred; hospice experience preferred.

Responsibilities

• Coordinate and direct hospice services per physician orders and patient plan.
• Participate in Interdisciplinary Team (IDT) meetings and follow hospice policies.
• Provide nursing care per current rules and guidelines; document accurately for billing.
