Patriot Healthcare is seeking an RN Case Manager to coordinate and direct hospice services, collaborating with an interdisciplinary team to provide compassionate patient care. The role emphasizes nursing process adherence, proper documentation, and liaison with physicians and facility staff.

Qualifikationen

Must possess a Nursing Degree from an accredited college or university.
Registered Nurse license in the State of South Carolina.
1 year of nursing experience preferred (hospital, long-term care or related health care facility).
Hospice experience preferred.

Aufgaben

Coordinate and direct hospice services per physician orders and patient plan of treatment.
Participate in Interdisciplinary Team (IDT) meetings and follow hospice policies.
Document care accurately to support professional billing and compliance.
Supervise hospice aides and CNA schedules as needed.
Maintain up-to-date patient records and communicate with physicians and facility staff.
