You will work with an interdisciplinary team to ensure compassionate, high-quality care, while documenting care and supervising aides as needed.

Qualifications

• Must possess a Nursing Degree from an accredited college or university.
• Licensed as a Registered Nurse in the State of South Carolina.
• 1 year of inpatient/hospital or long-term care nursing preferred; hospice preferred.

Responsibilities

• Coordinate and direct hospice services per physician orders and patient plan.
• Serve as a member of the Interdisciplinary Team (IDT) and attend meetings.
• Follow hospice policies, nursing standards, and state/federal regulations.
• Document direct patient care for accurate billing and care planning.
• Provide supervision of Hospice Aides and CNAs as needed.
• Communicate with physicians, facility staff, and family members to coordinate care.
