Job summary

Patriot Healthcare is seeking an experienced RN Case Manager to coordinate and direct hospice services per physician orders and patient plans. The RN will work within the interdisciplinary team to ensure compassionate care, documentation, and compliance across all nursing activities.

The role requires SC RN licensure, a nursing degree, and preferred hospice or hospital/long-term care experience. This is a full-time on-site position in Greenville, SC, with comprehensive benefits and supportive

Qualifications

• Nursing degree from an accredited college or university and SC RN license.
• One year of nursing experience in a hospital, long-term care facility, or related health care facility preferred.
• Hospice experience also preferred.

Responsibilities

• Coordinate and direct hospice services per physician orders and patient plan.
• Serve on Interdisciplinary Team (IDT) and attend meetings.
• Maintain nursing standards and documentation for billing and compliance.
• Supervise Hospice Aide and CNA schedules.
• Report to Director of Clinical Services as needed.
