Job summary

Patriot Healthcare is seeking an exceptional RN Case Manager to join our hospice care team. As part of the interdisciplinary team, you will coordinate and direct hospice services following physician orders and patient plans, ensuring comfort and dignity at life’s most important moments.

You will document care, supervise aides, and communicate with physicians to coordinate resources. A South Carolina nursing license and a nursing degree are required, with hospital or long-term care experience

Qualifications

• Must possess a Nursing Degree from an accredited college or university.
• Must be licensed as a Registered Nurse in the State of South Carolina.
• 1 year of hospital or long-term care nursing experience preferred; hospice experience preferred.

Responsibilities

• Coordinate and direct hospice services per physician orders and patient plans.
• Work as part of an Interdisciplinary Team (IDT) to provide compassionate care.
• Maintain accurate patient records and documentation for billing and quality assurance.
• Support facility nursing staff and supervise aides as needed.
• Communicate with physicians and team members to coordinate care.
