Job summary

Continuum Hospice is seeking a Full Time Hospice RN Case Manager (RNCM) to join our nursing team providing end-of-life care throughout the community. Schedule: Mon-Fri, 8 am - 4:30 pm with on-call rotation. Territory: Warwick/Coventry, RI.

You will deliver hands-on hospice nursing, coordinate services, and guide patients and families through their hospice journey with compassion, clinical excellence, and connection.

Qualifications

• Current unencumbered RN license.
• Minimum 1 year RN experience in medical-surgical/acute care; hospice exp a plus.
• Must be computer proficient in typing and various programs, including EMR background.
• CPR certification current if required by state.
• Valid driver's license, vehicle insurance, and dependable transportation.

Responsibilities

• Empower Through Engagement: respond to referrals with care and expertise.
• Collaborative Care Champion: work with Clinical Director to assess eligibility for hospice services.
• Nurturing Direct Care: deliver hands-on nursing care and coordinate services.
• Nursing with Heart: provide care rooted in proven nursing principles.
• Educator and Advocate: educate patients and families to participate in care plans.
• Call of Compassion: participate in on-call duties.
