Job summary

Lighthouse Hospice is seeking a compassionate Registered Nurse Case Manager (RNCM) to join our team in delivering exceptional end‑of‑life care across Mercer, Burlington, Camden, Gloucester & Salem counties. The role offers a Monday–Friday schedule, with a 8 am–4:30 pm shift and an emphasis on patient comfort, family support, and collaboration with the interdisciplinary team.

Qualifications

• RN license current and unencumbered.
• Minimum one year RN experience in medical-surgical/acute care; hospice a plus.
• Must be proficient with computers and EMR systems.
• CPR certification if required by state.

Responsibilities

• Respond to referrals with care and expertise, addressing inquiries compassionately.
• Collaborate with Clinical Director to assess client eligibility for hospice services.
• Provide direct nursing care to patients ensuring comfort and quality of life.
• Deliver nursing care grounded in established principles to ease pain and provide solace.
• Educate patients and families to empower participation in care plans.
• Participate in on-call duties as part of the team.
