Overview

Make a lasting difference as a Hospice Registered Nurse Case Manager (RNCM)! Sanctuary Hospice is seeking a Full‑Time Hospice RN Case Manager (RNCM) to join our dedicated nursing team providing exceptional end‑of‑life care throughout the community.

Schedule: Monday – Friday, 8 am – 4:30 pm and On Call As Needed.
Territory: Rockford and surrounding counties – Winnebago, Boone, McHenry, Dekalb, Ogle, Stephenson, and Lee.

Compensation

$89,000 – $96,000 per year (Average Pay Range). The pay range listed is a general guideline and is not a fixed offer. Final compensation will be determined based on experience, qualifications, and specific responsibilities and will be discussed at the conclusion of the interview process.
