Job summary

Moments Hospice is seeking an experienced RN Case Manager to lead clinical care for patients in the Illinois service area. This full-time, field-based role focuses on symptom management, care coordination, and family support, delivering compassionate end-of-life care in the patient’s home or preferred setting.

You will conduct comprehensive assessments, develop individualized plans, educate families, and collaborate with the care team. A strong commitment to dignity and comfort is essential.

Qualifications

• Active RN license in the state you work in
• Proficiency with electronic medical records (EMR)
• Strong organizational, communication, and critical-thinking skills
• Ability to work independently while maintaining high standards of care
• A compassionate, patient- and family-centered approach
• Valid driver's license and reliable vehicle with current insurance

Responsibilities

• Complete initial and ongoing comprehensive assessments of patients' physical, emotional, and psychosocial needs
• Develop, implement, and update individualized plan of care in partnership with patients and families
• Provide skilled nursing care, including pain and symptom management, medication administration, and comfort measures
• Educate patients and caregivers on disease progression, symptom management, medications, and what to expect at end of life
• Coordinate care across your caseload and serve as the primary liaison between patients, families, physicians, and the care team
• Participate in interdisciplinary group (IDG) meetings to ensure comprehensive, holistic care
