Job summary

Transitions Hospice in Illinois is seeking an experienced RN Case Manager to serve as the clinical anchor for patients and families, coordinating care, guiding the interdisciplinary team, and showing up when it matters most. You will conduct comprehensive assessments, build care plans with physicians, administer treatments per orders, and ensure accurate documentation.

This role requires compassionate, independent nursing in a hospice setting.

Qualifications

• Active RN license in the state of employment.
• Graduate of an accredited school of Nursing.
• One (1) + year Hospice, home health or related organization experience preferred.
• Compassion for patients and families navigating end-of-life care.
• Strong clinical judgment and the ability to work independently in the field.
• Excellent communication skills across care teams and families.

Responsibilities

• Coordinate care as the clinical anchor for patients and families.
• Guide the interdisciplinary team and respond when it matters most.
• Conduct comprehensive, ongoing assessments of patient needs.
• Build and adjust personalized care plans with physicians and care teams.
• Administer medications and treatments per physician orders.
• Maintain accurate, timely, and meaningful clinical documentation.
• Serve as the communication bridge between families, physicians, and community health partners.
• Educate patients and caregivers to feel confident and supported.
• Participate in on-call rotation per team policy.
