Job summary

Affinity Health Management is seeking an experienced registered nurse to lead hospice care in home and community settings. The role emphasizes comprehensive assessments, careful care planning, and coordination with physicians to meet patient needs.

Qualifications

• Graduate of an accredited school of nursing.
• Licensed to practice as a Registered Nurse in good standing.
• Minimum 1 year of professional nursing experience within last 3 years; hospice/home health preferred.
• Supervision of hospice aides not required but may be involved.

Responsibilities

• Perform initial and ongoing comprehensive patient and family assessments.
• Develop and initiate plans of care with goals based on nursing diagnosis.
• Administer medications and treatments as prescribed and document outcomes.
• Counsel patients and families on care, self-care and health maintenance.
• Coordinate patient care with the interdisciplinary team and physicians.
• Participate in on-call duties and ensure continuity of care.
• Maintain accurate clinical notes and communicate changes to the physician.
