Job summary

Heartland Hospice in Illinois is seeking a dedicated Registered Nurse to serve as an RN Case Manager. You will guide patients and families through the hospice journey, delivering skilled nursing care at patients' homes or other settings, and coordinating with an interdisciplinary team to refine care plans.

The role emphasizes education, compassionate support, and alignment with individualized plans of care. A $5,000 sign-on bonus is available for eligible candidates.

Qualifications

• Active RN license in state of employment.
• Minimum 1 year RN experience (hospice/home health preferred).
• CPR certification.
• Ability to educate patients and families.

Responsibilities

• Perform comprehensive patient assessments and reassessments.
• Deliver and document skilled nursing care per Plan of Care.
• Coordinate with Interdisciplinary Team for care plans.
• Provide education and emotional support to patients and families.
