Qualifications

• Active RN license in the state where you work.
• Minimum 2 years of nursing experience; hospice experience preferred.
• Valid driver's license and CPR/BCLS certification.
• Compassionate, patient- and family-centered care with strong communication.

Responsibilities

• Coordinate in-home hospice care for patients and families.
• Educate caregivers on care plans and symptom management.
• Collaborate with the care team to provide comfort, peace, and dignity.
• Document patient status, progress, and outcomes.
