Job summary

HHCP is seeking a compassionate Registered Nurse to provide specialized hospice care, guiding patients and families with tailored plans of care and expert symptom management. As Case Manager, you will coordinate services, educate families, and collaborate with physicians to ensure comfort and quality of life in the home setting.

You will join a team focused on geriatrics, home health, hospice and palliative medicine, with a strong emphasis on advocacy, coordination, and compassionate care

Qualifications

• RN licensure in the state with active CPR certification.
• Bachelor’s degree and 1+ year hospice experience preferred.
• 1–2 years of acute care experience in an institutional setting, plus 1–2 years of home care.
• Strong observation, problem-solving, and communication skills to advocate for patients.
• Ability to lift, position, and transfer patients; prolonged standing, walking, bending.

Responsibilities

• Conduct comprehensive, ongoing assessments of patients to determine physical, emotional, and hospice-specific needs.
• Serve as the dedicated Case Manager, designing, initiating, and regularly revising personalized plans of care.
• Provide high-quality nursing care with a focus on pain and symptom management for optimal comfort.
• Empower and counsel patients and their families, providing health education and emotional support.
• Communicate changes in condition to physicians, obtain orders, and collaborate in meetings.
• Supervise and support hospice aides to ensure high standards of care.
• Participate in the on-call rotation for urgent patient support.
• Coordinate medical equipment and services for the home setting.
