Job summary

UnitedHealth Group is seeking a Registered Nurse Case Manager to oversee the Plan of Care for hospice patients and collaborate with the care team to promote physical, psychosocial, emotional, and spiritual well-being. Responsibilities include initial evaluations, care coordination, and documentation, with work focused on diverse home settings and ensuring quality of care for each patient and family.

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• Current driver's license, valid vehicle insurance, and access to a dependable vehicle or public transportation.

Responsibilities

• Perform initial nursing evaluation to determine hospice eligibility within 48 hours of referral.
• Identify patient/family needs and re-assess every 15 days or as needed.
• Initiate and coordinate the plan of care; document progress and outcomes.
