Job summary

UnitedHealth Group is seeking a Registered Nurse Case Manager for Assured Hospice. You will deliver the Plan of Care for hospice patients, collaborating with the IDG and other professionals to promote physical, psychosocial, emotional, and spiritual well-being.

The role requires compassionate care for patients and families facing end-of-life decisions. Responsibilities include initial home evaluations, coordinating care plans, and maintaining detailed clinical notes.

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• 1+ years of clinical experience.
• Current driver’s license, valid vehicle insurance, and access to dependable transportation (or public transport).
• CPR certification preferred or within 90 days of hire.
• Able to work independently.
• Strong communication, writing, and organizational skills.

Responsibilities

• Initial nursing evaluation within 48 hours of referral to determine hospice eligibility.
• Identify patient/family needs and re-assess at least every 15 days.
• Initiate and coordinate the plan of care.
• Document problems, goals, interventions, and responses to care.
• Collaborate with patient/family, attending physician and IDG daily.
• Instruct patient/family in self-care techniques when appropriate.
• Maintain accurate and relevant clinical notes.
• Operate effectively in diverse home environments regardless of demographics.
